Privacy Policy
Effective Date: March 2026
Your Privacy Matters
Chris Platania, LPC ("we," "our") is committed to protecting the privacy of all individuals who use this website. This policy describes what information we collect, how we use it, and how we protect it.
Information We Collect
Contact form submissions: When you use our contact form, we collect your name, email address, phone number (optional), and message. This information is used solely to respond to your inquiry.
Intake and assessment forms: If you are directed to complete a form by your counselor, the information you provide is protected health information (PHI) governed by HIPAA. We describe these protections below.
We do not use cookies for tracking, analytics, or advertising. Session cookies are used only for authenticated staff access and are required for security.
HIPAA Compliance
As a licensed counseling practice, we comply with the Health Insurance Portability and Accountability Act (HIPAA). This means:
- All protected health information is encrypted in transit (TLS/HTTPS) and at rest.
- Access to client records is restricted to authorized staff only.
- All access to protected health information is logged in an audit trail.
- Sessions automatically expire after a period of inactivity.
- We maintain a Business Associate Agreement (BAA) with our hosting provider.
- Public forms (intake, assessments) use unique, time-limited access tokens — no login or account creation is required.
How We Protect Your Information
- All data is transmitted over encrypted HTTPS connections.
- Our database is encrypted at rest with industry-standard encryption.
- Strict Content Security Policy and security headers prevent common web attacks.
- Public forms do not use autocomplete or browser suggestions to prevent accidental data exposure.
- No personal health information is ever included in URLs, error messages, or application logs.
Third Parties
We do not sell, share, or disclose your information to third parties except as required by law. Our hosting infrastructure is provided by Google Cloud Platform under a signed Business Associate Agreement.
Your Rights
You have the right to request access to, correction of, or deletion of your personal information. For any privacy-related requests, please contact us at 434-760-0428 or chris@chrisplatanialpc.com.
